  • Type of plan requested-employee or employer paid.

  • The group's location.

  • The producer's marketing plan- with voluntary programs, it is especially important to include one-on-one enrollment meetings.

  • The ages, occupations and gender mix of the group.

Don't waste valuable time submitting a disability income application. To provide a starting point for determining disability income insurance eligibility, use these questions first to pre-screen your prospective clients:
  1. What are your duties at work?
  2. Do you work in your home? If yes, what percentages of your duties are there?
  3. Do you participate in any hazardous avocations?
  4. Are you self-employed? For how long? How many employees do you have? What is your percentage?
  5. Do you use any tobacco products?
  6. Is your height and weight average?
  7. Do you have any significant health history and are you taking any medications?
  8. Have ever taken an antidepressant medication or received counseling for any reason
  9. What is your taxable earned income for the last 2 years?
  10. Do you now have group or individual disability income coverage?
